Clients are required to complete and return three documents before commencing riding with RDASA. Payment of Rider Registration fee (currently $50) must be made at the same time.

These mandatory documents are:

Medical Consent Form (with current year's date)
Appendix 5
Rider Registration Form (with current year's date)

 

Also included here is an information leaflet for riders which outlines important information regarding privacy, advocacy, complaints, service access & exit and rights and responsibilities.

Sometimes riders come for a 'one-off' ride to try RDA so need to complete a 'one-off ride permission form'  before they ride.
