Clients must complete and submit three important documents to their Centre before they are allowed to ride with RDA SA.  Payment of Rider Registration fee (currently $50) must be made at the same time, as well as any session fees required by their Centre.

These mandatory documents are:

Medical Consent Form - must be signed by your doctor or specialist
Membership Declaration (Appendix 5)
Rider Registration Form (with current year's date) 

Also included in the Rider Registration form is a leaflet containing important information about client privacy, advocacy, complaints, service access & exit and rights and responsibilities.  Read this carefully and retain a copy for your reference.

Sometimes riders come for a come-and-try ride with RDA; they must complete a 'one-off ride permission form'  before they ride.
